| Implementation of Van Service
1. Counselor identifies customer needing driving evaluation/training service.
2. Customer obtains Pennsylvania learner's permit unless they have a license.
3. Customer has the Medical Approval for Driver Evaluation (1.3MB .pdf) completed by
the same physician that completes the permit application. The form is then faxed or sent
to Brant's Driving School.
4. Counselor contacts Brant's Driving School to set up an evaluation.
5. Evaluation is scheduled and completed at the customer's home or another convenient
location
6. Evaluation report is generated within two days and forwarded to the referring counselor.
7. If the customer is recommeded for driver training, it is scheduled as soon as approval
is given and time permits.
8. Driver training is completed in Brant's Driving School's van.
9. Driver testing is done in Brant's Driving School's van.
10. Adaptive driving equipment prescription and structural modifications are written and
forwarded to the counselor and Transportation Service Coordinator.
11. Van modifications are required to have a final checkout which is attended by the
Transportation Service Coordinator, vendor, customer and Certified Driver Rehabilitation
Specialist.
12. Follow-up training is completed in the customer's van after the checkout is complete.
